Creatine, Protein and Working Out on Accutane
If you’re considering Accutane (isotretinoin), you might be wondering how it could affect your gym or exercise habits. Is creatine safe? Why do your joints ache? Can you keep lifting at all? Can You Take Creatine on Accutane?
There is no known direct interaction between creatine and isotretinoin. The FDA label says to avoid or be cautious with things like vitamin A supplements, tetracycline antibiotics, and some more specific medications like systemic corticosteroids and anticonvulsants. Creatine isn’t listed.

Creatine does, however, raise creatinine levels on bloodwork. The kidneys themselves are fine; it’s more that the marker itself becomes slightly skewed by the supplement. Creatinine isn’t a standard measurement for isotretinoin lab requirements, but it’s often included by default, so you shouldn’t be too worried if you see those levels raised.

What Creatine Does and Doesn’t Do to Your Kidneys
So, to be clear, creatine doesn’t impair kidney function in healthy people, and the raised creatinine markers don’t mean kidney dysfunction.

And again, none of this relates specifically to isotretinoin. Being on isotretinoin means getting bloodwork at least once, so a raised creatinine is more likely to be noticed, especially because everyone is looking for abnormal results.
Can You Keep Working Out on Accutane?
Another question you might have is whether you can keep going to the gym or working out while on Accutane (isotretinoin). The answer is yes: you can continue to exercise, work out and go to the gym while on isotretinoin.
The one side effect that might change how exercise feels is muscle and joint aches, which around 16% of patients experience during treatment. They’re generally mild to moderate and clear rapidly after you stop the medication, but some patients stopped treatment early because of them, and in rare cases they persisted.

How much you feel the aches can depend on the type of activity, as well as your dose and how long you’ve been on treatment. Some activities strain your joints more than others, especially during an isotretinoin course. In my experience, running and lifting weights felt normal, but after training martial arts I could definitely feel aches in my back that I wouldn’t normally get to that degree.
Back to bloodwork: creatine kinase (CK, also written CPK) is a marker of muscle strain, and vigorous activity may raise your CK levels while on isotretinoin. The significance is unknown, and rare cases of rhabdomyolysis have been reported, some during strenuous activity. In one trial, 12% of adolescents had a temporary CK elevation, with half returning to normal within two weeks and the rest within four weeks, and no cases of rhabdomyolysis.

So it’s not that you should avoid strenuous activity or hard training, but you should know the warning signs of rhabdomyolysis: severe muscle pain, weakness and dark-colored urine. In fact, exercising and maintaining a healthy, balanced diet can be beneficial not only to your overall health, but to your skin and acne results as well.

Is Protein Powder OK on Accutane?
Speaking of diet, is protein powder OK? Protein powder is not an isotretinoin interaction, and there’s no evidence or literature suggesting otherwise.

Just as important, though, is water intake. Dryness is a huge isotretinoin side effect, and training can dehydrate you even more, potentially making those symptoms worse. Keep up your hydration.
