How Much Does Accutane Cost in 2026?
If you’re seriously considering Accutane (isotretinoin), one of the major things you’re probably thinking about is price, and the answers online are genuinely confusing: “packages”, “per month”, “insurance”, etc., and all three can be true at the same time because a treatment course has different moving parts.
This guide breaks down the cost into components, gives dated cash prices for the medication, explains how insurance works into everything, adds up what six-month courses by route look like, and explains how to potentially pay less. Everything is dated because these prices can change over time.
How Much Is Accutane Without Insurance?
As of October 2026, a 30-capsule fill of generic isotretinoin 30 mg lists at $220.13 retail on GoodRx, with coupon prices from $55.27 at CVS to $103.49 at Costco.
That is the medication only; a full course without insurance adds prescriber visits, lab bloodwork draws, and, for patients who can become pregnant, monthly pregnancy tests.
GoodRx (2026). Isotretinoin 2026 Prices, Coupons & Savings Tips
Things like pharmacy choice, time, difference between retail sticker and cheapest coupon price, and other variables can drastically impact the price you end up paying.
The Cost Components People Forget
An Accutane (isotretinoin) course really has five cost lines, but most price pages only mention the first:
- The capsules: Dispensed 30 days at a time with no refills, so you pay per month for medication over the length of the course.
- Prescriber visits: An initial evaluation and then a check-in every month, because iPLEDGE requires a new prescription each cycle. In an office, this is a visit charge each time; online services either charge per visit or bundle it.
- Lab work: A fasting lipid panel and liver enzymes before you start, with more draws only if your prescriber wants them.
- Pregnancy tests: For patients who can become pregnant: two before starting, one before every monthly dispense, one at the end and one 30 days after the last dose, as the program is written today.
- Time: Travel, missed work or school, and the cost of a missed window. One study showed roughly three hours of school and $31 to $41 in indirect costs were lost every month to follow-up visits.

Generic Isotretinoin Cash Prices
| Fill | Source and date | Retail or average price | Coupon or discount price |
|---|---|---|---|
| Isotretinoin 30 mg, 30 capsules | GoodRx, 4 Oct 2026 | $220.13 | $55.27 (CVS, Target) to $103.49 (Costco); Walgreens $76.26, Walmart $102.98 |
| Isotretinoin 30 mg, 60 capsules | SingleCare, 4 Oct 2026 | $1,285.33 average | $80.23 (CVS) to $199.07 (Walmart); Walgreens $159.77 |
| Isotretinoin 40 mg, 60 capsules | ClearHealthCosts reporting GoodRx, Jul 2023 | n/a | $105.40 (Rite Aid) to $919.68 (Walgreens) |
Two things to note here: one, the “retail” price is close to fiction for most people; almost nobody pays $220 or $1,285 once a free coupon is in hand. Two, the gap between chains is persistent: CVS has been at the bottom and Walgreens or Walmart near the top in each of the snapshots above, three years apart.
A coupon covers the capsules only. You still need a certified prescriber, monthly check-ins, lab work and, for patients who can become pregnant, pregnancy tests.
Why Price per Milligram Is Not Linear
Isotretinoin is priced per capsule, not per milligram. A 10 mg capsule and a 40 mg capsule cost the pharmacy roughly the same to buy and dispense, so a 20 mg daily dose taken as two 10 mg capsules can cost nearly twice what the same dose costs as one 20 mg capsule, and a 40 mg dose as a single capsule can be the cheapest milligram you buy.
Practically, your prescriber can write the dose in the fewest capsules that make clinical sense, and ask the pharmacy to price the alternatives. Dose decisions belong to the prescriber, but the capsule count itself is often flexible.
Brand vs Generic: Absorica, Claravis, Zenatane, Amnesteem
Brand Accutane left the US market in 2009. Everything dispensed today is either a generic (Claravis, Zenatane, Amnesteem, and Myorisan are the common labels) or the brand Absorica and its lower-dose sibling Absorica LD, which come in 10, 20, 25, 30, 35, and 40 mg capsules.
The generics are bioequivalent to the original and are what the cash prices above refer to. Absorica is formulated to absorb better without a high-fat meal, which is a real convenience for some patients, but it is priced as a brand and is rarely the economical choice. Your prescriber may have a reason to pick it; price alone is not one.
One underrated cost factor is absorption. The label notes that taking isotretinoin with food roughly doubles absorption compared with an empty stomach. Capsules taken without food are, in effect, money spent on a lower real dose, and a lower real dose is one of the things that raises the chance of needing a second course.
Is Accutane Covered by Insurance?
Most commercial plans and state Medicaid programs cover generic isotretinoin, but almost all of them require prior authorisation, and the criteria are stricter than the clinical guideline.
A typical commercial policy requires “severe, recalcitrant nodular acne unresponsive to conventional therapy,” plus trial and failure of one systemic antibiotic in combination with one generic topical retinoid used consistently for three months.
Initial authorisation lasts five months, but a second five-month course can be approved if severe acne persists or recurs and at least two months have passed since the first course ended.
Priority Health (2018). Pharmacy Prior Authorization Form: Oral Isotretinoin
The wording varies by plan (some Medicaid programs accept a one-month antibiotic trial, some commercial plans want two topical classes), but the shape is the same everywhere: nodular acne, documented failures, five months at a time.
Compare that with the 2024 American Academy of Dermatology guideline, which strongly recommends isotretinoin for acne that is severe, causing psychosocial burden or scarring, or failing standard therapy.
Insurers are still writing based on the older “severe nodular” standard, which is why a dermatologist can recommend isotretinoin for moderate, scarring acne and an insurer can still deny it. If your acne is moderate rather than nodular, or you have not completed a three-month antibiotic-plus-retinoid trial, expect a denial or a request for documentation, and expect your prescriber’s notes and photos to matter.
With insurance that does authorise it, the medication copay is often modest, but the visits and labs fall under your medical benefit, with their own copays or deductible.
HSA and FSA
Prescription isotretinoin, the prescriber visits that manage it, and the required lab work are all qualified medical expenses! That applies to cash-pay telehealth too, including Happyglow. Pay with the HSA or FSA card where you can, and keep itemised receipts.
What Labs Cost Without Insurance
Isotretinoin can raise triglycerides and nudge liver enzymes, which is why the label calls for blood tests before and during treatment. The question is how often.
Monthly draws for the whole course used to be routine, and plenty of practices still schedule them, but the research has moved on: a large review of 26 studies found no support for monthly testing at standard doses, and a 2022 expert panel recommends checking only before starting and at peak dose.
Lee et al. (2016). JAMA Dermatology
Your prescriber sets the lab schedule, and more frequent checks are the right call for some patients (high doses, abnormal baselines, diabetes, obesity, heavy alcohol use). But if you are quoted monthly labs as a non-negotiable, it is fair to ask why.
That matters for your wallet because each draw has a price: about $35 to $50 a panel at direct-to-consumer labs as of October 2026, and several times that at a hospital outpatient lab.
At Happyglow, your physician orders a baseline panel before you start, and repeats it only if your results or dose call for it, although this varies by case and where the current guidelines are at.
How Much Is Accutane for 6 Months? Total Cost by Route
The table assumes six monthly cycles, generic isotretinoin, one baseline lab draw for Happyglow (lab schedules on other routes vary), and (where relevant) seven pregnancy tests. Prices are as of October 2026.
| Route | What you pay for | Approximate 6-month total |
|---|---|---|
| Dermatology office, cash | Initial visit plus 5 to 6 follow-ups, drug at coupon price, labs, pregnancy tests | Varies widely; often several thousand dollars (see the published figures below) |
| Dermatology office, insured | Visit copays, drug copay after prior authorisation, labs under deductible | Widely variable; often a few hundred dollars to about $1,500 |
| Online dermatology service, pay per piece | Consultation fee (one service charges $500 for the year), drug at coupon price, labs at a direct lab | About $900 to $2,100 |
| Happyglow, Low-Dose Isotretinoin | $199 per month: physician care, prescription, monthly check-ins, medication and free shipping | $1,194, plus a one-time baseline lab and any pregnancy-test fees |
| Happyglow, Standard Isotretinoin | $229 per month: same inclusions, 20 mg or 40 mg, set by your physician based on your weight | $1,374, plus a one-time baseline lab and any pregnancy-test fees |
For the office route, the reference point from published data rather than market estimates is the Shah cohort above, where the average total treatment cost was $3,246.53 for male patients and $2,736.91 for female patients, with medication the largest component.
Where Happyglow’s Price Sits and Why
Happyglow’s isotretinoin is $199 per month for the Low-Dose protocol (10 mg or 20 mg) and $229 per month for Standard (20 mg or 40 mg).
That is a flat fee per 30-day cycle covering the physician’s evaluation, the monthly check-in, the prescription, the medication, and shipping. If the physician decides isotretinoin is not appropriate for you, the fee is refunded in full.
Put that next to the price table above: at some pharmacies, a month of capsules alone can cost about the same even with a coupon, and that is before anyone has paid for a prescriber, a check-in or shipping.
The price follows the protocol, not the milligrams: each protocol is one flat monthly fee for the full course of care, whatever capsule strength your physician prescribes in a given month.
Against the table, that puts Happyglow well under the cash dermatology route, in the same band as piecing together an online consult, coupons, and labs yourself, and above a fully insured course with low copays.
Honestly, if you have insurance that will authorise isotretinoin and a dermatologist you can see monthly without missing work, that could be your cheapest option. Happyglow is a streamlined one, with the physician, the check-ins, and the medication in one monthly line, and a baseline lab ordered to a draw site near you.
How to Lower the Cost Legitimately
There are legitimate ways to lower your cost for Accutane (isotretinoin):
- Use a coupon every month: the retail-to-coupon gap is the largest saving available, and it applies to every monthly fill.
- Price the capsule count: the same daily dose in fewer, larger capsules is usually cheaper. Ask the pharmacy before you fill.
- Compare chains: in both the 2023 and 2026 snapshots above, the cheapest and dearest major chains were a factor of two to eight apart for identical capsules.
- Use a direct-to-consumer lab: about $35 to $50 a panel versus hospital outpatient pricing, and ask your prescriber how many draws you actually need.
- Take it with food: the label’s absorption data means an empty stomach wastes part of every capsule.
- Do not miss a pickup window: in one US study, patients who missed even one window ended up with longer courses and about $940 more in total costs.
- Finish the course properly: stopping early to save a month or two is a false economy if it means a second course later. More on that in Is Accutane Permanent?
What Not to Do
Don’t buy isotretinoin from a website or vendor that doesn’t require a prescription, or isn’t licensed. Those aren’t cheaper versions of the same medication: they’re unregulated products from unknown sources, and nobody takes responsibility if something goes wrong. The red flags are listed in How to Get Accutane Online, and our telehealth safety article covers how to check a platform.
The legitimate route can cost a bit more and take more steps, but iPLEDGE, licensed physicians and pharmacies, and regulated platforms are what help keep treatment safe and on track. Happyglow is LegitScript certified, and our physicians and pharmacies are licensed and regulated in the United States. If something goes wrong, we’re here to help.

Common questions
How Much Is Accutane Without Insurance?
As of October 2026, a 30-capsule, 30 mg fill of generic isotretinoin lists at about $220 retail on GoodRx, with coupon prices from roughly $55 at some chains to just over $100 at others. Add prescriber visits, at least one lab draw and, for patients who can become pregnant, monthly pregnancy tests. A full six-month course without insurance typically lands somewhere between about $900 and $4,800 depending on the route.
Does Insurance Cover Accutane?
Most commercial plans and Medicaid cover generic isotretinoin, usually with prior authorisation. Insurers typically require a diagnosis of severe, recalcitrant nodular acne and a documented failure of an oral antibiotic plus a topical retinoid over about three months, and approve five months at a time. If you qualify, the drug copay is often modest; visits and labs are billed separately under your medical benefit.
How Much Does Accutane Cost per Month?
Budget for three things each month: the capsules (about $55 to $220 for a 30-day generic fill at cash or coupon prices as of October 2026), the prescriber check-in (free to a few hundred dollars depending on the route), and any lab or pregnancy test due that month. Flat-fee online services bundle the visit and the medication; Happyglow is $199 or $229 per month depending on protocol.
Is Generic Accutane Cheaper Than Brand?
Yes, by a wide margin. Brand Accutane was discontinued in 2009; what pharmacies stock now are generics (often labelled Claravis, Zenatane, Amnesteem or Myorisan) and the brand Absorica and Absorica LD. The generics are bioequivalent and cost a fraction of the brand price, so unless your prescriber has a specific reason for Absorica, generic is the default.
Can You Use an HSA or FSA for Accutane?
Yes. Prescription isotretinoin, the prescriber visits that manage it and the lab work it requires are qualified medical expenses, so HSA and FSA funds can be used for them, including for cash-pay telehealth services. Keep itemised receipts in case your plan administrator asks.
What Is the Cheapest Way to Get Accutane?
The cheapest route is usually generic isotretinoin at a coupon price, labs at a direct-to-consumer lab rather than a hospital, and the fewest visits the program allows. If you have insurance that will authorise it, a covered prescription plus a dermatologist with low copays can be cheaper still. The most expensive mistake is missing an iPLEDGE window, which stretches the course and adds visits and tests.
